O stars! My brother was here after a hospital stay after a brain tumor recection. He has stage 4 lung cancer with mets. He was better when he arrived there then when I "demanded" discharge this past Thursday. He had a nasty cough for a whole week and when I picked him up for doctors appointments he was so weak and sick! Nobody addressed it. Asked the house doctor if she even examimed him or listened to his lungs when she visited. My brother said she never did. He called me crying every night. Ignored by staff. He couldnt sleep because the loud staff roaming hallways all night like they were having a dang party. This morning after I brought him home on Thursday, he went into respiratory failure. We rushed him to hospital and found out he had pneumonia. Had to intubate. He's in bad shape in ICU now. These places are supposed to rehabilitate you, not make you worse!! DO NOT SEND YOUR LOVES ONES HERE!!!!!! WORSE FACILITY EVER! I worked in Healthcare for years and been a caregiver for all my loved ones and this place is by far the worst place I've experienced. His Oncology and Radiation nurses where shocked by his condition. 10 days and now he's in ICU. There was a few good staff members there. If I was them, I'd look elsewhere for employment. I'd NEVER want to be associated with a place like this. I feel so sorry for the residents. They all look so sad and miserable
DO NOT SEND YOUR LOVED ONES HERE!!! My MIL just was admitted on 5/3/25. When she was in the hospital she was on 2-4L of O2. Once at this place she had no O2 being ran and we had to fight in order to get it. Just oher 48 hours later she fell out of bed when we all had asked to get guard rails for the bed or a bed alarm or something. They said that was considered "restraint" and they couldn't do that. She is a fall risk and they had to have known since she was still confused from her aneurysm. After she fell they claimed that there was no indication she was hurt. We requested she be sent for eval like every other home does when a fall happens and was not witnessed. They insisted she was okay but we had her sent anyway. Her BP is tanked, she has a massive black eye and blood around her mouth. Who knows what else. This is awful. Poor excuse for a facility overall if this is how the residents are cared for. If this progresses and something happens we will be pursuing further action against this facility for negligence.
If I could leave a zero star I would. They allowed my mother, with documented dementia, to AMA herself when they found out POA paperwork was incorrect. Encouraged me to allow her to be sent to a psychiatric facility which had zero impact on her Dementia symptoms and discharged her based on " they could not accommodate her anymore". Arranged for transportation home after she signed discharge paperwork. I had asked twice for the AMA document TWICE to be sent home with discharge paperwork which was not included. Upon her arrival by medical transportation, EMT stated that they were told I AMAed her. I never signed anything, nor would I, I had been at work ALL day until her arrival home. There was very little physical therapy provided in the skilled facility which was the whole purpose of the stay. Then the very next day, had the AUDACITY to call Adult Protective Services reporting that she lives by herself..she was at home (with my brother and I) TWO hours before she fell and was admitted back into a hospital. If there wasn't a discharge or AMA, this could have been avoided. However, due to the extreme negligence of this facility, I was granted emergency guardianship. I have filed a formal complaint with Medicare which will demand they show the AMA paperwork. I am looking forward to them failing the investigation. Be very careful in considering this facility, they are experts at being deceitful and falsifying information.
I would never recommend this facility to anyone. ZERO Stars. The staff literally blocked the hallway so my sister could not be taken to her room by EMTs. It was late Sunday night (Super Bowl) and even though she was returning from test being performed, by their request, from the hospital. The treatment was unprofessional and appalling, as they denied access smiling and smirking.
The Manor at Perrysburg is a great place for rehab and long term stays! The staff is great, always willing to go above and beyond. The therapy gym is very nice and the therapists are knowledgeable and personalize your family members care. My dad came after a stroke last year, ended up staying for several months before he passed. I am grateful they took care of him and treated us like family :)
