Meridian Home Health Care is Nurse owned and operated Medicare certified adult home health agency providing Skilled Nursing, Physical Therapy, Occupational Therapy, Speech Therapy, Home Health Aide and Medical Social Workers to the geriatric and elderly community, through our highly trained and qualified professionals. When a patient transitions to our care, our team works closely with our patient's physician to prevent hospital re-entry.
