Registered Nurse (RN) Job Description
Responsibilities: Perform and coordinate field and telemed visits as needed by regulation and necessary by the patient. Educate the patient and family/caregiver about hospice service to comply with the plan of care per diagnosis. Train and education patient, family/caregivers and staff on appropriate usage of medications, DME, and supplies. Confirm physician, caregivers, family, hospice team members are informed about change in condition. Oversight to compliance with all documentation on time, on schedule, and complete. Be familiar with Government regulation and guidelines. Complete HIPPA and all training in company. Coordinate and participate in Interdisciplinary Team meeting and discuss and implement intervention or changes timely. Participates in On-Call rotation as necessary as primary coverage and coordination of support staff. Performs job in compliance with Agency policies, procedures and applicable laws. Supervisory, or management of employees, including experience with scheduling. Comfortable with technology, use of tablet for email, telemed, admin docs, and cell phone. Able to lift, transfer and when necessary bathe or assist in bathing patient. Use of leadership skills to assist new staff on the field and build skills and team confidence. Qualifications: Ability to communicate effectively with clients, families, caregivers, and administrative staff. Confirmed leadership and interpersonal skills as well as flexibility and while working within of a high standard of stress. You are passionate, ambitious, eager to improve and computer-literate. 2+ years working in Home Care or related services. Valid Driver’s License.
