Caregiver for an Assisted Living Facility

Basic duties assisting with personal care, bathing and grooming, dressing, toiletting, and exercise. Basic food preparation, preparing meals, housekeeping and laundry. Help with getting in and out of a wheelchair, car or shower. Provides constant companionship and general supervision. Requirements: Complete the LiveScan Request Form (Form LIC 9163) Take it to an approved LiveScan vendor and submit your fingerprints. Visit the California Department of Justice website for a list of approved LiveScan vendors in your area. CPR and First Aid Statement of Prior Criminal Records (LIC 508) TB clearance within 90 days prior to application or within 7 days after completing your application. Health Care form clearance
