This role assures that the organization is in compliance with state licensing requirements and all applicable state and federal regulatory standards are continually met. Responsibilities: ​ -Regularly assesses the needs of clients receiving Nursing services -Assess and chart observation of the patient’s condition at each visit -Dress or redress wounds and assess the healing process -Provide education to patients and families on proper home health care procedures and strategies. -Coordinate with Physicians, Physical Therapist, Occupational Therapists, and other individuals in the patient’s care plan -Provide hands on care, (i.e., wound care, ventilator care etc.) -Recommend tools or devices that may improve quality of life for the patients -Regular evaluations and reevaluations (as warranted by changes in condition but at least every 60 days) of the patient’s nursing needs -Taking verbal orders -Make referrals to other disciplines, as indicated by patient’s assessment -Maintain a current and accurate
