(408)429-8665 www.cschomecare.com Job Description: -Provide routine individualized care to seniors or persons with disabilities -Monitor and report changes in client's health status -Assist with non-medical daily activities which include: companionship, light housekeeping, meal preparation, transportation, medication reminders, personal care, dementia supervision, etc. -Drive clients/errands -Be on call as a Caregiver. -Be able to do full transfers, stand by assist, safety supervision -We are looking for reliable and caring individuals!
