Registered Nurse (RN) Job Description
The prime responsibility of the RN is to provide direct care and supervision for each consumer; and supervision over Direct Support Professionals (Certified Nursing Assistants); and supervise other allied medical support staff. This job classification has been identified as having a risk for acquiring an occupational exposure to blood or other potentially infectious materials. Protective equipment such as gloves, gowns, masks/face shields should be worn when exposure to blood or other potentially infectious materials is anticipated. Clinical judgment should be exercised when deciding what type of protective equipment to wear. Responsibilities include, but are not limited to the following: 1. This position is a mandated reporter, and is expected to observe all consumers rights, report unusual occurrences and suspected dependent adult abuse, as specified in Title 22, California Code of Regulations. 2. Performs and/or directs nursing procedures. 3. Responsible for starting, monitoring or administering medication into an intravenous (IV) line. 4. Maintains confidentiality and compliance with HIPAA. 5. Maintains the health of persons served by conducting physical assessments, monitoring health and dietary needs. 6. Prepares psychiatric and physical assessments, performs nursing diagnosis, and health care objectives. 7. Responds to emergencies that involve use of medical/behavioral intervention techniques. 8. Provides supports to ensure all basic needs. 9. Initiates CPR and first aid as needed. 10. Participates with consumers on appointments, activities and general outings in the community as indicated. 11. Assists consumers in all activities of daily living skills using teaching/coaching and mentoring techniques. 12. Assists staff in occupational, recreational, vocational and educational programs for consumers. 13. Assures the delivery of health-related services and provides nursing services to maintain health, maximize independence and support. 14. Oversees care and conducts training and on-going development by providing day to day supervision of direct care staff. Monitors the activities and documentation of other staff for accuracy and completeness and follow up as necessary. 15. Participates as a member of the Interdisciplinary Team in all aspects of the development and implementation of the Individual Program Plan. Evaluate and revise healthcare status and health care plans to address all priority healthcare needs. Assists staff in occupational, recreational, vocational and educational programs for consumers. 16. Observes and documents consumers’ physical condition and behavior and reports significant changes to appropriate staff. 17. Communicates regularly, and as necessary, with the family member(s), guardian(s) and significant others of persons served. 18. Works with consumers in activities that provide opportunities for learning and practice of skills tailored to the specific needs of the individuals. 19. Provides one-to-one supervision/direct observation of consumers on special occasions and precautions, i.e. an outing activity, elopement risks, suicide risks and medical condition and risk of serious self-injurious behavior to client. 20. Coordinate all healthcare-related appointments and other activities; provides transportation and accompanies persons to and from appointments and activities. 21. Evaluate each consumer’s status to ensure that needs are being met in a continuous manner. 22. Maintains a safe, sanitary, and therapeutic environment which promotes respect and dignity and protects the privacy, rights, confidentiality and physical/emotional well-being of the consumers. 23. Responds to emergencies that involve the use of medical/behavioral intervention techniques. 24. Participates with consumers on appointments, activities and outing in the community as scheduled. 25. Works extended hours and/or varying shifts. Works at other various homes operated by LSA, within Santa Clara County, as needed.
