Home Health Aide

We are looking for 1 - 2 licensed CNAs to join our team at Divinity Home Care as a Home Health Aide. We are a Medicare A certified Home Care Agency, providing services throughout Macomb County. Hours are flexible, and you have the ability to arrange visits around your schedule! Pay starts at $20 per visit plus mileage, which is reimbursed for required travel outside of to your first patient and home from your last patient. Responsibilities: * Work with intake and case managers to be given a list of patients to see each week. * Reach out to the patients to schedule visits to meet frequency. * Assist with showers and adls for home bound patients. * Work with other care team members (RN, PT, OT, SLP) to instruct patients on how to perform tasks more independently before being d/c from home care.
