Home Health Care RN

We are seeking an experienced RN to join our Medicare A Certified Home Health Care team in Macomb County, providing patient care and case management to our team of PTs, OTs, HHAs, SLPs and LPNs! Divinity Home Care offers flexible schedules, competitive pay per visits, mileage reimbursement, and many other benefits to our part time and full time employees! You make your own schedule each week - so long as patient frequencies are met! Weekend coverage is required once a month to open new patients as needed, and increased weekend rates are offered. Responsibilities: Administer nursing care to ill, injured, or disabled patients Diagnose and establish patient treatment plans along with case managing Monitor and report changes in patient symptoms or behavior Communicate with collaborating physicians or specialists regarding patient care Educate patients about health maintenance and disease prevention Facilitate referrals to other healthcare professionals and medical facilities Maintain accurate patient medical records Provide advice and emotional support to patients and their family members Qualifications: * 1 year experience in home care setting preferred with knowledge of Medicare A requirements! * Ability to navigate through user friendly EMR (DeVero) and complete ALL notes within 48 hours of treatment.
