Licensed Practical Nurse (LPN) Job Description
Minimum Qualifications: Associate in Science Degree in Nursing; Must be a Licensed Practical Nurse (LPN) by the State of Florida and maintain an active license; Possess a (valid, eligible) Driver License, proof of automobile insurance and access to private, reliable, daily transportation. Three years of experience providing direct services in a hospital, long-term care facility or social service agency are required. Additional coursework in a related behavioral science is beneficial. Job Title: Licensed Practical Nurse NATURE OF WORK: Under the supervision of the REGISTERED NURSE the ADULT DAY CENTER LICENSED PRACTICAL NURSE is responsible for the following tasks: 1. Provide nursing care in an adult day care setting. 2. Supervise program’s adult day care aides and other appropriate staff. 3. Assist Registered Nurse with reports. 4. Complete State of Florida Department of Elder Affairs comprehensive assessments, upload entries to the Client Information and Referral Tracking System and maintain client files. 5. Observe, report, and record both normal and abnormal signs and symptoms of assigned clients. 6. Document treatment and client progress on a monthly basis. 7. Review clinical files to ensure that required documentation is up-to-date. 8. Administer assigned medications per physician’s orders and documents on Medication Administration Record. 9. Change designated dressings per physician’s orders. 10. Perform blood sugar monitoring with an Accu-chek meter; report and record results per physician’s orders. 11. Administer insulin therapy and breathing therapy per physician’s orders. 12. Irrigate wounds per physician’s orders. 13. Prepare and administer subcutaneous and intramuscular therapy per physician’s orders. 14. Assist clients on a daily basis with Activities of Daily Living. 15. Collaborate with the serving of breakfast and lunch as needed per the Adult Care Food Program guidelines. 16. Operate and maintain special equipment (i.e. nebulizers; portable oxygen; weight scales; PEG feeding). 17. Participate in programmatic functions/activities. 18. Display professional demeanor. 19. Display appropriate communication skills. 20. Abide by established work schedule. 21. Ensure that all service activities are performed per accreditation, funding sources and State of Florida Agency For Health Care guidelines; and assist with developing and implementing Performance Quality Improvement (PQI) activities. 22. Adhere to Program, Department and County policies and procedures and safety guidelines. 23. Participate in professional development trainings. 24. Monitor quality of services being provided. 25. Attend scheduled in-service training sessions. 26. Perform other related duties as required.
