Home Care CNA: Floater 3P-11P Mon-Fri & e/o Weekend

CNA Floater Job Description: • Stay on campus during down time when not with a Home Care client to assist floor staff in health care areas • Work with a team to provide excellent resident centered care • Work with multiple clients in varying areas at Well•Spring Life Plan community (Clients will vary between Home Care from Well•Spring Solutions clients and residents at Well•Spring Life Plan Community) • Immediately report and record changes in client condition, family situation, or needs to Supervisor • Follow specific written and verbal instructions regarding client care duties, following client care plan • Provide second assist to floor staff at Well•Spring Life Plan Community • Provide laundry services and light housekeeping • Assist with preparation or pick-up of meals • Monitor diet and nutritional status • Aid with eating, grooming dressing, mobility (walking), toileting and incontinence care, and with transferring • Aid with communication and correspondence by phone or mail • Aid with maintaining client schedules and calendars • Escort for appointments on and off campus • Aid with hobbies and interests • Escort to and aid with activities at Well•Spring Life Plan community • Perform other related duties as directed by Home Care Admin and Nurse Supervisors
