Registered Nurse (RN) Job Description

A Home Health Registered Nurse (RN) provides direct patient care in a home setting, ensuring quality healthcare services while promoting independence and well-being. The RN works closely with patients, families, and healthcare teams to develop and implement personalized care plans.
Key Responsibilities:
• Conduct in-home assessments to evaluate patients’ medical needs, conditions, and environments.
• Develop and implement individualized care plans in collaboration with physicians and healthcare professionals.
• Administer medications, wound care, and other prescribed treatments.
• Monitor patients’ progress and document assessments, interventions, and outcomes.
• Educate patients and families on disease management, medication adherence, and self-care strategies.
• Coordinate with therapists, social workers, and other caregivers to ensure comprehensive patient care.
• Maintain compliance with healthcare regulations, policies, and procedures.
• Respond to emergency situations and provide critical nursing interventions as needed.
Job Requirements:
Education & Licensing:
• Active Registered Nurse (RN) license in the state of practice.
• Associate or Bachelor’s Degree in Nursing (ADN/BSN) from an accredited program.
• BLS (Basic Life Support) and CPR certification (ACLS may be required for some roles).
Experience:
• Previous experience in home health, hospice, or acute care (preferred but not always required).
• Strong ability to work independently and make critical decisions in a home setting.
• Experience with wound care, IV therapy, medication administration, and chronic disease management is a plus.
Skills & Competencies:
• Excellent communication and interpersonal skills to build strong patient relationships.
• Strong organizational and time-management skills to handle multiple patient visits per day.
• Proficiency in electronic medical records (EMR) documentation.
• Ability to lift, move, or assist patients as needed.
• A valid driver’s license and reliable transportation for home visits.
