Licensed Practical Nurse (LPN) Job Description
PRINCIPAL FUNCTIONS Provides skilled nursing care to patients as prescribed by the Physician, under the guidance and supervision of a Registered Nurse. Gives care in a manner, which promotes quality, continuity, and safety for the patient. DUTIES AND RESPONSIBILITIES 1. Provides nursing care to clients through implementation of the nursing care lam written by a registered nurse. 2. Contributes to the formulation/revision of the nursing care plan under the direction and supervision of the Clinical Supervisor or his/her designee. 3. Implements orders for medications and treatments as prescribed by the patient’s Physician and all aspects of the nursing care plan. 4. Assists the Physician and Registered Nurse in performing specialized procedures, preparing equipment, and materials for treatments. 5. Assists clients with activities of daily living while promoting the highest level of independent functioning available. 6. Instructs patients in self- care techniques, or other aspects of care as ordered by the Physician or outlined in the nursing care plan. 7. Works with family members and other members of the health care team to meet emotional needs of the client and family members 8. Evaluates and maintains a safe environment for the patient. 9. Observes the patient for changes in physical and mental condition and reports these changes to the appropriate Physician, Registered Nurse/Case Manager and/or Clinical Supervisor. 10. Completes, maintains, and submits accurate and relevant clinical notes regarding the patient’s condition and all care and treatments given. 11. Seeks and obtains sufficient information necessary to function effectively in the health care setting assigned. 12. Accepts assignments and performs duties which are within the limits of preparation and experience. 13. Participates in patient case conferences. 14. Will submit to supervised field visits at least monthly. 15. Will submit to annual performance review. 16. Assumes responsibility to maintain and upgrade professional knowledge and skills through attendance and participation in continuing education and in-service programs. 17. Will receive and review a copy of the Saguaro Job Description and relevant policies and procedures for home health care and agrees to apply these to the care of any Saguaro patients taken under care. QUALIFICATIONS 1. Current Licensure as a Licensed Practical Nurse 2. At least one year of experience working as a Licensed Practical Nurse, preferably within the year prior to the date of application for employment. 3. Proof of CPR certification (this must be obtained by the employee). 4. Evidence of a pre-employment physical exam or certification from a licensed physician within 12 months prior to hire, that the person is in good physical health, is free from active tuberculosis and any other communicable disease in a contagious stage; or a copy of an annual health assessment if employed longer than one year. 5. Record of a two-step Mantoux tuberculin skin test prior to employment and an annual one-step Mantoux tuberculin skin test thereafter. An employee with a documented history of a positive Mantoux tuberculin skin test is exempt from screening with skin tests, and is followed under the TB surveillance protocol, per policy. 6. Must submit to a fingerprinting screening (criminal background search).
