Experienced Hopsice RN Case Manager Multiple Territories DFW Metroplex

Pathway Hospice, the leading locally owned hospice provider in DFW, is seeking an experienced RN to join our wonderful team as a Hospice Case Manager. The ideal candidate will be dedicated to providing exceptional care for our patients and their families. While our office is in Richardson, TX, we are currently recruiting for several DFW territories. Pathway offers an excellent benefits package, including but not limited to: Competitive Salaries Company pays 70% of Employees’ Medical Insurance Paid Time Off Company Car Responsibilities of this position include the following: Develops, coordinates, and follows the patient's individualized plan of care Case manage total care of patient by coordinating with team members/physicians, supervising CNAs and participating in staff and Interdisciplinary Team meetings Assess patient’s physical condition according to Plan of Care; determine and/or revise plan of care regarding medications and DME Appropriately identifies/utilizes resources: Dietary, Pharmacy, Supplies, Home Medical Equipment, etc. Serves as a patient and family advocate; Maintains focus on the patient and family experience of care Completes documentation in a timely manner Provides education to patients, families, and caregivers Participates in patient care meetings as assigned Communicated effectively with coworkers and patients/family Participates in bereavement support Job Type: Full-time
