Field RN Case Manager

We are expanding our area!! We are looking for Hospice LVNs & RNs who are able to provide an outstanding, compassionate level of care in the Greenville, Kaufmann, Wills Point, Athens, Quinlan, Terrell and other areas East of Dallas. LVNs would be responsible for: Perform skilled nursing visits as needed. Coordination of care with the LVNs, HHAs, Clinical Manager. Coordination of care with physician offices Attend IDT Meetings. Provide communication via phone and patient comm notes Timely Documentation Knowledge of Healthcare First or other Hospice software preferred. Bilingual is a huge PLUS!!! RNs would be responsible for: · Completing comprehensive assessments, · Perform supervisory/skilled nursing visits as needed. · Perform Pronouncement visits. · Assist with On Call on occasion · Coordination of care with the LVNs, HHAs, Clinical Manager. · Coordination of care with physician offices · Attend IDT Meetings. · Provide communication via phone and patient comm notes · Timely Documentation · Knowledge of Healthcare First or other Hospice software preferred. Bilingual is a huge PLUS!!!
