Certified Nursing Assistant (CNA) Job Description
We are a large, ever growing and expanding facility looking for the right Health Care Professional to join our team assisting our Health Services Director with the initial assessments, re-assessments, and event investigations. As well as promoting the health and well being of the Assisted Living residents and contribute to the smooth operation of the department. Duties: As directed, perform initial assessments for new residents. Able to provide smooth intake and admission of new residents to Assisted Living programs. As above, perform re-assessments for the annual review or change of condition, MMSE, skin and other required components of the overall assessment/re-assessment process. Understand the negotiated service agreement and be able to make changes as needed. Assist with the resident service plan as directed. Proficient with all required documentation, this includes but not limited to MMSE, skin assessments, fall risk, event report, and event investigations, MARS and physician or family conference, meetings, and calls. Direct with the assistance, monitoring and recording of medications and treatments. Proficient with medication pass. Ensure that medications are available by obtaining physicians orders and completing pharmacy requisitions. Proficient with the MARS process Monitoring health status of AL residents in the AL programs Consult with the health care providers regarding the health of the residents, as needed. Function as a liaison with physicians, hospitals, families, ancillary health service, hospice, and private caregivers Maintains all resident records. Responsible for ordering lab work, obtaining or directing the obtaining of specimens, recording appropriately. Documentation, assessment of/or observations for falls, bedrails, UTI’s, dehydrations, weight loss, neuro checks, change of conditions, safety checks, wound care, dressing changes, colostomy care, etc. Practices confidentiality setting an example for staff and residents. Assist the Director of Health Services in organization or delivery of staff meetings, in-services, and orientations. Is knowledgeable and understands the WACs for Washington State Boarding homes. Assists the Health Services Director in employee reviews and survey process. Other duties as assigned. All experienced, reliable, and dedicated LPN's, MA's, MSW's and NAC's encouraged to apply.
