Humancare Home health Care Agency has immediate opening/s for certified Home health aides (HHA's) in Rockland and Orange County assisting patients with daily living needs of all ages and medical conditions
Position Title: Home Health Aide "HHA"
Job Summary: As a Home Health Aide, you will provide essential care supporting patients of all ages in their homes. Your responsibilities will include assisting with daily living activities, monitoring health conditions, and maintaining accurate records.
Responsibilities:
Assist patients with personal care tasks, such as bathing, dressing, and grooming.
Help patients with mobility, transfers, and ambulation.
Administer medications as directed by healthcare professionals.
Monitor vital signs and report any changes or concerns promptly.
Provide companionship and emotional support to patients.
Maintain accurate documentation of patient care activities.
Collaborate with other healthcare team members to ensure comprehensive care.
Qualifications:
Valid Home Health Aide certification (New York State Certified).
Proof of legal residency or citizenship.
Compassionate, patient, and reliable.
Ability to follow care plans and adapt to individual patient needs.
Excellent communication skills.
Bilingual (English, Spanish or Creole) is a plus for serving culturally diverse clients.
Must have patient care experience
