The Certified Nursing Assistant is responsible for observing the patient, reporting these observations to the RN and documenting observations to the RN and documenting care. Essential Job Functions: 1. Gives personal care including baths, back rubs, oral hygiene, shampoos and changing bed linen as often as assigned. 2. Assists in dressing and undressing patients as assigned. 3. Assists in feeding patient as assigned. 4. Takes and records oral, rectal, and axillary temperatures, pulse respiration, and blood pressure when ordered with appropriate completed/demonstrated skills competency. 5. Provides proper care and observation of patient’s skin to prevent breakdown of tissue over bony prominence. 6. Reports on patients’ condition and significant changes to Registered Nurse. 7. Assists in ambulation and exercise as assigned by a Registered Nurse. 8. Offers and assists with bedpans and urinals. Helps with light laundry needs. 9. Keeps patients living area clean and orderly, as assigned. 10. Adheres to the organization’s documentation and care procedures and standards of personal and professional conduct. 11. Participates in personal, professional growth and development. Also participates in quality assessment performance improvement and attend IDT meetings. 12. Respectful of patient and family/caregiver environment and patients’ personal needs. 13. Performs personal care needs after given a patient’s assignment and attending to his/her requests promptly. If unable to perform a certain task, report to the Registered Nurse. 14. Meets safety needs of patient and using equipment safely and properly (foot stools, side rails, O2 etc.) 15. Respects confidentiality and privacy of all patients, caregivers, and fellow team members. Communicates in a respectful and courteous manner. 16. Minimizes non -productive time and fills any periods of low census and/or slow periods with activities and cross-training that will enable you to meet the future needs of the organization. 17. Seeks opportunities for professional growth, including demonstrating the ability to receive constructive performance feedback. Attends at least twelve (12) hours of continuing education programs per calendar year. 18. Licensed driver with automobile that is insured in accordance with organization requirements and is in good working order. Must be willing to travel the entire service area. 19. Other duties as assigned.
