Qualifications • Must be 18 years of age or older (Required) • License/ Certification / Certificate: RN, LPN, CNA, HHA, STNA (Required) • (2) forms of identification (Required) • High School Diploma/GED (Required) • Current Physical (Required) • Current TB test (Required) • Current First Aide & CPR certification (Required *Resources for training available) • Proof of Covid-19 Vaccination (Preferred) • BCI (Required) • Reliable transportation (Required) • Smart phone, Android or iPhone device (Required) • Valid email address (Required) • EVV experience (Preferred) • Experience with (EMR) electronic medical records (Preferred) Responsibilities • Acknowledgement and adherence to established company policies and procedures • Performs personal care and homemaking task as directed on the patient’s care plan • Timely completion of visit documentation and EVV requirements • Ensures safety of patient’s while performing all tasks and assignments, reports changes in the patient’s condition and needs to the agency R.N. Supervisor • Participate in patient care conferences • Assist patient with medication reminders • Errands and light grocery shopping as directed on the patient’s care plan • Assist patient with mobility and physical therapies/exercise, e.g., range of motion (ROM) and ambulation as assigned • Performs additional task as needed directed on the patients care plan COVID-19 considerations: As essential employee you would be required to follow the CDC guidelines to include the Agency Covid-19 policy. PPEs would be provided
