Job Description: 1. Establishes and maintains an effective personal hygiene routine for the participant. 2. Maintains a clean and safe environment for the participant in his/her home. 3. Prepares meals in compliance with prescribed diet. Consults with dietician as needed. 4. Encourage participant to participate in his/her own care taking into consideration participants physical limitations. Checks with supervisor for special instructions. 5. Assist with self-administration of medications. 6. Provides emotional support to participants. 7. Reports any pertinent findings/changes in participants condition to supervisor. 8. Is dependable and follows Bienvivir Senior Health Service policy and procedure. 9. Cooperates and works closely with supervisor. 10. Performs other tasks/duties as delegated by Home Health RN Supervisor. Qualifications: 1. Must be 18 years or older. 2. Must have minimum one year experience caring for the frail elderly. 3. Ability to understand and follow specific instructions. 4. Ability to read and write English. 5. Experience in personal care and home management. 6. Bilingual preferred. 7. History of ethical behavior, no criminal record.
