Home Health Aide (HHA) Job Description

We are currently looking to hire Registered Nurses, Licensed Practical Nurses, Certified Nurse Assistants, Home Health Aides, Physical, Occupational Therapists and Assistants. Qualifications: • 1 to 2 Years of experience • Professional Florida license • Current vehicle and Personal liability insurance • CEU'S • CPR • Bilingual preferred, not required • Excellent people, verbal and written communication skills • Ability to pass AHCA Background Check • Caring and Compassionate RN/LPN • Complete initial and ongoing assessments to identify the physical and environmental needs of patients. • Complete assessments at appropriate time points. • Regularly re-evaluate the patient's nursing needs and evaluates the outcome of care. • Assists patients in recovery or maintenance of life functions; prevent illness and relapse of illness. • Develop, initiate, and revise the Plan of Care as necessary to ensure quality and continuity of care. • Initiate appropriate preventative and rehabilitative nursing procedures. Plans for discharge of the patient from services. • Provide effective communication to patients, their families, and other health care professionals. • Participate in Case Conference. • Apply agency's infection control plan and universal precautions in coordination and execution of client care. • Coordinate services and inform physician and other personnel of changes in the patient's conditions and needs. PT/PTA • Performs and documents initial and ongoing assessments of patient’s condition. • Designs and implements a plan of care which is appropriate to issues identified, required frequency of treatments, and involves the patient/family. • Educates and counsels patients and families regarding treatment plans and progress. • Provides information regarding appropriate selection/use of adaptive equipment and support resources. • Supervises activities of physical therapy assistants, students and other support personnel. • Plans and/or assists with patient discharge from therapy CNA/HHA • Provide service to Elderly, Disabled, Special Needs Children and Adults • Provide patient care under the direction of Supervisor • Assist in the transport of patients • Provide companionship and basic care to patients • Maintain a clean and healthy environment • Light Housekeeping • Respite Care • Homemaker • Companion
