Home Health Aide in Santa Clara County

As a member of the interdisciplinary team, the Certified Home Health Aide provides personal care services to patients and performs related tasks in the patient/family place of residence in accordance with the plan of treatment as assigned by the RN Case Manager. Coverage Areas needed: Santa Clara County - Milpitas / San Jose / Santa Clara / Campbell / Los Gatos / Saratoga / Cupertino / Sunnyvale / Mountain View / Los Altos / Palo Alto / Gilroy / Morgan Hill Duties: -Duties assigned are to increase the level of comfort, and to maintain personal hygiene and a safe, healthy environment for the patient and to provide for temporary respite and relief from total care responsibilities for the primary caregiver -Perform other duties as assigned -Monitor patient condition by observing physical and mental condition, intake and output, and exercise -Support patients by providing housekeeping and laundry services; shopping for food and other household requirements; preparing and serving meals and snacks; running errands -Assist patients by providing personal services, such as, bathing, dressing, and grooming -Help patients care for themselves by teaching use of cane or walker, special utensils to eat, special techniques and equipment for personal hygiene -Help family members care for the patient by teaching appropriate ways to lift, turn, and re-position the patient; advising on nutrition, cleanliness, and housekeeping -Record patient information by making entries in the patient journal; notifying nursing supervisor of changing or unusual conditions -Maintain a safe, secure, and healthy patient environment by following asepses standards and procedures; maintaining security precautions; following prescribed dietary requirements and nutrition standards -Adhere to professional standards, home care policies and procedures, federal, state, and local requirements Required: CHHA Certification/License
