Licensed Practical Nurse (LPN) Job Description

As a team member with Lakeview Terrace, you will provide services to our residents that call LVT home. Facility Name and Address: Lakeview Terrace Retirement Community 331 Raintree Drive Altoona, FL 32702 Purpose: Maintain documentation and complete necessary forms reflecting residents’ conditions and treatments. Observe and report symptoms and condition changes of residents to physicians, director of nursing, and/or designee, resident’s representative, or other 3rd party providers, as necessary. And above all, provide excellent patient care. Essential Duties: Assume responsibilities as Community Nurse, when applicable, and respond to emergency calls of residents within the Independent Community. Understand and properly use the facility communication system, including the emergency call system. Follow physicians’ orders and perform accurate and timely administration of medications; complete appropriate paperwork, and maintain documentation in residents’ charts.. Provide care utilizing infection control measures. Complete 24-hour report on a daily basis and advise supervisor of actions taken. Give report to on-coming shift using 24-hour report and communication book to convey important information. Supervise Certified Nursing Assistants during assigned shifts. Secure and store properly all toxic/hazardous substances utilized in the work area. Be aware of and utilize proper procedures in disposal of all such substances. Demonstrate proper body mechanics to prevent injury to self or residents. Adhere to HIPAA guidelines for resident confidentiality and promote and protect the residents’ rights. Complete required trainings; Attend all required in-services and departmental staff meetings. Ensure compliance with all federal and state rules and regulations. Complete other duties as assigned by supervisor. Required Qualification: Current license as a licensed Practical Nurse in the State of Florida and IV certification. CPR certified For purposes of effective communication with residents, families and staff must be able to see, hear, speak, read and write English. Maintain ability to stretch, push, pull, kneel, reach, twist, lift unassisted at least 50 lbs and walk or stand up to eight hours.
