Licensed Practical Nurse (LPN) Job Description
Primary Responsibilities
• Provide direct nursing care to residents according to their care plans.
• Administer medications and treatments accurately and timely.
• Monitor residents for changes in condition and promptly report findings to the RN or unit manager.
• Assist with ADLs and help with lifting, transferring, and repositioning residents when needed.
• Perform routine procedures such as wound care, catheter care, G-tube care, and vitals.
• Maintain accurate and timely documentation of all care, observations, and resident changes.
• Ensure compliance with all infection control and safety protocols.
• Respond quickly and appropriately to emergencies, including performing CPR if necessary.
• Assist with admissions, discharges, and transfers, including documentation and assessments.
• Follow physician orders and notify the nurse or unit manager of any discrepancies or concerns.
Collaboration & Communication
• Communicate with residents, families, and interdisciplinary team members in a respectful and professional manner.
• Participate in care plan meetings as requested.
• Ensure continuity of care by providing thorough shift reports and updates.
• Notify the supervising nurse of any resident or staff concerns.
Team Leadership
• Step in and help STNAs with care tasks when needed (e.g., transfers, hygiene, feeding).
• Provide guidance, mentoring, and on-the-job training to STNAs as needed.
• Act as a role model and maintain a professional, compassionate attitude with all staff and residents.
Additional Duties
• Keep med carts, supply areas, and resident rooms clean and organized.
• Attend and participate in mandatory in-service training and education.
• Adhere to the employee handbook, including all policies on attendance, behavior, and safety.
• Float to other halls or units as needed, based on facility needs.
• Carry out any additional duties as assigned by the unit manager or DON.
