Licensed Practical Nurse (LPN) Job Description
Responsible for conducting non-medical home assessments and completing comprehensive head-to-toe evaluations for clients receiving in-home care services: Initial, 30-days after services begin, and quarterly.
This role supports quality assurance, client safety, and compliance with agency and regulatory standards.
• Identify potential safety risks, skin issues, or health concerns and report findings promptly to the Clinical Supervisor or Case Manager.
• Communicate effectively with caregivers, clients, and agency staff to ensure continuity and appropriateness of care plans.
• Uphold confidentiality and professionalism at all times while representing the agency in the community.
Physical Requirements: • Ability to travel to client homes and perform physical assessments. • May require occasional lifting, bending, and standing during client visits.
Qualifications: • Active Licensed Practical Nurse (LPN) • Strong assessment and documentation skills. • Excellent communication and critical-thinking abilities. • Reliable transportation and valid driver’s license required. • Ability to work independently and manage time efficiently.
Work Schedule: • PRN / As Needed Flexible scheduling based on agency needs and client referrals.
