Care Finders Total Care, a Modivcare Personal Care Service, is looking for an RN or LPN responsible for providing high-quality services to our home care private duty clients.
We are seeking a compassionate, skilled LPN or RN to provide one-on-one nursing care for a 13-year-old child with Spastic Cerebral Palsy in a home setting. The ideal candidate is patient, attentive, and experienced in pediatric and/or neurological care, with a strong commitment to supporting the child’s medical needs, comfort, safety, and overall quality of life.

Pay: LPN hourly rate $35 - RN hourly rate $45
Schedule: 1 day - Sunday 12 hr shift

Responsibilities
Provide direct nursing care in accordance with the child’s individualized care plan
Assist with activities of daily living (ADLs), including positioning, mobility support, hygiene, feeding, and toileting as needed
Monitor vital signs, neurological status, muscle tone, and overall health
Administer medications and treatments as prescribed
Perform range-of-motion exercises and support physical therapy goals as directed
Manage and monitor medical equipment (e.g., feeding tubes, suctioning equipment, braces, mobility devices, if applicable)
Recognize and respond appropriately to changes in condition or medical emergencies
Maintain accurate and timely documentation of care provided
Communicate effectively with parents/guardians, therapists, physicians, and care team members
Provide emotional support and promote a nurturing, respectful environment for the child
Follow infection control, safety, and privacy standards at all times
Qualifications
Current, valid LPN or RN license in good standing
Experience with pediatric patients and/or neuromuscular or developmental disabilities preferred
Knowledge of cerebral palsy and related care needs is a strong plus
CPR/BLS certification (pediatric preferred)
Ability to safely lift, transfer, and position a child with mobility limitations
Strong assessment, critical-thinking, and communication skills
