Responsibilities * Assist the Ad with efforts to improve interdisciplinary collaboration, ensuring the transition-of-care processes are executed smoothly with strong patient and family communication, and fostering a supportive team environment.* Act as a backup for AD in reviewing referrals.* Managing admissions, including entering new physician orders and completing all admit tasks except those requiring an RN assessment.* Ensuring rooms are ready for new residents and ensure that the customer serviceexperience is maintained at the start of admission to the facility.* Following up with residents post-discharge to ensure that discharge home is successful and if any barriers noted offer guidance/support.* Assisting with facility marketing/Liaison to our referral sources/partners* Participate in the on-call nursing rotation and providing floor support when there are no admits Qualifications * Minimum of one (1) years’ experience in a healthcare setting.* Minimum of LPN Licensure in the state of Pennsylvania.* Ability to adjust work schedule based on centers’ business need,specifically incoming admissions.* Must provide verification of TST (tuberculin skin test) as required by state lawand in accordance with Company policy. TSTs will be administered at the worksite if required.* This position requires that the employee is able to read, write, speak andunderstand the spoken English language to ensure the safety and wellbeing of ourpatients and visitors at the work site when responding to their medical and physicalneeds.
