We are currently looking for professional and skilled care staff that share and believe in what our company does and achieves. **REGISTERED NURSE Job Summary - A Registered Nurse administers skilled nursing to patients on an intermittent basis in their place of residence. This is performed in accordance with physician orders and plan of care, under the direction and supervision of the Director of Patient Care Services/Nursing Supervisor. Qualifications: 1. Graduate of an approved school of professional nursing and currently licensed in the state(s) in which practicing. 2. Once years nursing experience, preferred 3. Acceptance of philosophy and goals of this Agency. 4. Ability to exercise initiative and independent judgment. Some responsibilities include and are not limited to: - makes the initial evaluation visit and regularly reevaluates the patient’s nursing needs. - Initiates the plan of care and necessary revisions, appropriate preventive and rehabilitative nursing procedures - Coordinates services and informs personnel of changes in the condition and needs of the patient. **LICENSED VOCATIONAL NURSE Job Summary - A Licensed Vocational Nurse administers skilled nursing to patients on an intermittent basis in their place of residence. Graduate of a state approved school of practical (vocational) nursing and currently licensed in the state(s) in which practicing. 2. Minimum of one (1) year nursing experience, preferred. 3. Acceptance of philosophy and goals of this Agency. 4 Ability to exercise initiative and independent judgment. Some responsibilities include and are not limited to: - Implements the nursing care plan for each patient and initiates preventive and rehabilitative nursing procedures as appropriate. - Prepares clinical and progress notes and maintaining accurate and complete records of observations, treatments and care of patient. - Assists the patient in learning appropriate self-care techniques, while teaching and counseling the patient and family regarding nursing care.
