LVN CASE MANAGER - Office

We are seeking an in-office Case Manager to join our hospice team! You will assist with managing a patient case load, running benefits, placing equipment, ordering medications and much more. Entry Level Welcome to apply, we will train you! RESPONSIBILITIES: Consistently monitor case load for patients/family's needs Triage phone calls Monitor and report changes to director of nursing Communicate with collaborating physicians or specialists regarding patient care Educate patients about health maintenance and disease prevention Facilitate referrals to other healthcare professionals and medical facilities Maintain accurate patient medical records Provide advice and emotional support to patients and their family members QUALIFICATIONS: Previous experience in nursing or other medical fields (1-6 months) Familiarity with medical software and equipment (1-6 months) Ability to build rapport with patients Strong problem solving and critical thinking skills Ability to thrive in a fast-paced environment Current LVN license ****Please forward your resume to the following email: volunteer@legacyhospicecare.net along with your times of availability to interview. ****Please refer to Janice Salgado or Lisa Hindi, HR Manager
