Licensed Vocational Nurse (LVN) position --- **Job Title:** Licensed Vocational Nurse (LVN) **Location** Multiple locations **Company:** Willow Spring Hospice Care (A subsidiary of Optum Health) **Job Summary:** The Licensed Vocational Nurse (LVN) at Willow Spring Hospice Care provides compassionate nursing care to patients in the end stages of life. The LVN works under the supervision of a Registered Nurse (RN) and as part of an interdisciplinary team to support the physical, emotional, and spiritual needs of hospice patients and their families. **Key Responsibilities:** - **Patient Care:** Provide direct patient care, including monitoring vital signs, administering medications, performing wound care, and assisting with activities of daily living. Ensure comfort and dignity for patients in their homes or in inpatient facilities. - **Support for Care Plans:** Assist in implementing individualized care plans developed by the RN and the interdisciplinary team. Provide feedback on patient status and care needs to support adjustments to the care plan. - **Patient and Family Education:** Educate patients and families about care techniques, symptom management, and end-of-life care. Offer emotional support and guidance to help families through the hospice experience. - **Documentation:** Maintain accurate and timely documentation of patient care, observations, and interventions in compliance with hospice policies and regulations. - **Team Collaboration:** Collaborate with RNs, physicians, social workers, chaplains, and other team members to ensure comprehensive care. Participate in team meetings and contribute to care planning discussions. - **Crisis Response:** Assist in managing patient crises by providing appropriate interventions and communicating with the RN and other team members to ensure timely response. - **Compliance and Safety:** Adhere to all regulatory requirements and company policies regarding patient care, safety, and documentation. Participate in quality improvement initiatives as required. **Qualifications:** - **Education:** Graduate of an accredited LVN program. - **Experience:** Minimum of 1 year of nursing experience, preferably in hospice, palliative care, or home health settings. - **Licensure/Certification:** Current LVN license in the state of California. Certification in Hospice and Palliative Care is a plus. - **Skills:** Proficient in basic nursing skills, medication administration, and patient care techniques. Strong communication and interpersonal skills. Compassionate and patient-focused. - **Other Requirements:** Valid driver’s license and reliable transportation. Ability to travel within the service area to provide care in patients' homes. **Working Conditions:** - **Environment:** The role involves working in various settings, including patient homes, inpatient facilities, and other care environments. - **Physical Demands:** The position requires physical activity, including lifting, moving, or transporting patients. Frequent standing, walking, and driving are necessary.
