LVN Case Manager-office (Elk Grove)

This is a great opportunity to provide powerful training one on one with Patients and their families. The training would include working with other CHHA’s, RN case managers and LVN’s in thee office of Elk Grove, CA. The patients range from children to young adults. This would include training and teaching patients that have. -G-tubes -Cerebral Palsy - Functional disabilities - Needs of patient/family training. Ideal candidate will require the following: -Valid LVN License -2+ year experience (Preferred) -Current CPR card -Physical (within1year) -Negative TB (within 1 year) -Malpractice Policy (NSO) agency can set this up if you don't have one We really are looking forward to working with you to help us take care of family needs in the Hayward, Oakland, & San Leandro areas.
