LVN / RN Home Health Case Manager (FT)

ERG Home Health Provider, Inc. is currently seeking a LVN and RN to fill the available Case Management role. This is an in office position, submitting candidates must also be able to commute daily to our Glendale, Ca office. Location: Glendale, Ca Schedule: Full-time ( Monday - Friday) Hourly Rate: Based on experience Benefits: Offered after 90-days of employment Essential Job Duties: Handle and coordinate incoming calls related to patients, physicians, and agency services regarding physician orders, participant questions, and referrals Communicate with patients via telephone, and provide effective communication with nursing therapy, aide, social services, and physicians, regarding changes in participant/staff schedule, test results, etc. In collaboration with Home Care Services staff, track and monitor home care and hour scheduling In coordination with the Marketing Team, help with enrollment of prospective patients into the program Assist with staffing/scheduling activities, soliciting, and input from managers Participate in end-of-life care, coordination, and support All other Case management related job duties Job Requirements: Active/Current LVN or RN License with no restrictions Bilingual a plus Bachelor’s Degree preferred Minimum of three (3) years of case management or nursing experience in a clinical or home health setting Must have transportation with proof of active car insurance.
