PRN & Full time Field LVN Needed!

Primary function is to administer skilled nursing care, under the supervision of a registered nurse for patients in their place of residence, coordinate care with the interdisciplinary team and patient/caregivers. This position will promote patients' potential during their home health episode and collaborates with physician, clinical team and patient in preparation for discharge. Qualifications: · Licensed Vocational Nurse, per the state requirements. · Good communication skills to be able to relate professionally and positively to patients, caregivers, colleagues and physicians and strong clinical documentation skills including wound care and IV Infusion. · Ability to read, write, and speak the English language.
