Licensed Vocational Nurse (LVN) Job Description
California Hospice Services, Inc. is looking for a Licensed Vocational Nurse (LVN) / Case Manager in a Hospice setting. We believe in our team and your ability to do skilled work with us. Assist RN and/or health care provider in an examination, treatment, and scheduling visits of the patient. You will be responsible for providing direct patient care under the supervision of an RN for the hospice care program. Good organizational skills, help to coordinate home-based palliative care services, and reporting all identified needs Patient assessment and approved plan of care by the registered nurse, you implement the approved nursing and home care Assist physicians and all healthcare professionals when caring for patients, always giving a complete and detailed report on patient status at end of shift/assignment and Completion of medical notes & Case Notes. Complete patient registration process per outlined best practice, which includes, but is not limited to, scanning insurance, government-issued ID, obtaining EOB, and consent forms. Educate patients and families on illnesses, medications, preventative measures, and overall during the visit & on call. Maintain prescription inventory, follow up with supply orders, and screen for expiration dates. Track immunizations for compliance with federal and state law, and job corps requirements. Maintain accurate and up-to-date patient medical records. Assist with cerumen removal and nebulizer therapy procedures as ordered by the provider on duty. Track labs/testing to ensure the continuation of care and for billing purposes Ability to develop rapport and positive working relationships with all departments in a courteous manner in diversified situations. Maintain a working relationship with community partners. Answer phones, respond to questions, complete callbacks (including lab reviews), and return calls as necessary. Excellent administrative, communication, and organizational skill with high attention to detail. Ability to operate all equipment necessary to perform the job and to move or lift up to 50 pounds. Subject to standing, walking, sitting, bending, reaching, kneeling, pushing, and pulling. Basic computer skills About You · Must have an active state-issued license as an LVN in the state of California. Knowledge of infection control practices. Have at least one year of experience as an LVN. Valid California driver’s license with an acceptable driving record & auto insurance. Successfully pass any background check and drug test required on the contract.'
