The ideal candidate will have experience in long-term care and home health. The Care Coordinator will be responsible for coordinating and monitoring the delivery of services to clients, providing guidance and support to clients and families, and ensuring that all services are provided in accordance with established standards. This position requires excellent organizational skills, strong communication abilities, and the ability to work independently. Responsibilities: Coordinate and manage the long-term care of clients. Assess the needs of clients and develop care plans to meet those needs. Provide home health services such as medication management, wound care, and other nursing services. Monitor the progress of clients and adjust care plans as needed. Communicate with physicians, nurses, and other healthcare professionals to ensure proper care is being provided. Educate clients and their families on available resources and services.
