Licensed Vocational Nurse (LVN) Job Description
Well established Home Health Agency in Tujunga seeking LVNs to join our Home Health team in the greater Los Angeles area. This skilled nurse will be able to make visits within the Glendale, San Fernando, Pasadena, La Canada / La Crescenta, Tujunga and the surrounding area. The ideal candidate will be a responsible and well-trained professional able to give the best nursing care with little supervision. The goal is to promote patient’s well-being by providing high quality nursing care. Responsibilities • Contributes to the development of a plan of care based on existing problems, expected patient response, and the Plan of Care. • Performs duties consistent with the State of California Vocational Nursing Practice Act • Implements safe, therapeutic care of patients with overt needs in supervised/ controlled situations as initiated in the plan of care by the physician in consultation with the case manager/ RN and the interdisciplinary team. • Explains test, procedures, disease process and provides health education to patient, family, and or/ caregiver. Documents patient/ family/ caregiver response to care and teaching. • Reports on patient’s condition and changes in patient condition to the Case manager/ Clinical supervisor • Prevents spread of infection and disease by proper disposal of contaminated materials and adhering to Standard Precautions • Assists patient in learning appropriate self-care techniques as appropriate. Skills • Must have at least 1 year working experience as a LVN. • Proven experience as a LVN. • Excellent knowledge of nursing process methods and procedures. • Ability to act appropriately and safely in emergency situations. • In-depth knowledge of health and safety guidelines and procedures (sanitation, decontamination etc.) and willingness to follow them at all times • A team player with excellent communication and interpersonal skills • Responsible and compassionate • Outstanding organizational and multi-tasking skills • Identifies problems and utilizes excellent problem-solving skills. • A graduate of an approved school of nursing who is licensed by the state where practicing. • Valid nursing license
