Full Time Hospice Nurse

**PLEASE DO NOT APPLY IF YOU ARE NOT A LICENSED NURSE IN CA We are a Hospice Agency in Valencia that is looking for a self motivated, compassionate RN or LVN Hospice Patient Coordinator. The job requires the ability to utilize a PC, Printer, multi-line telephone, and other related office equipment. LVN Will work from office. We do not offer work remotely. Responsibilities Schedule is Monday-Friday 8:30am-5pm. Coordinates the patient’s and families care and services including those of the team, social worker, home health aide and physician Responds necessary information to the Director of Nursing Monitors the re-certification / re-authorization process Ensures and documents appropriate use of the medications, durable medical equipment and supplies Educates patients, families and other caregivers Appropriately utilizes the skills and resources of contract personnel such as PT, OT, Podiatrist and Pharmacy Provides appropriate care plan and integrates care plans as needed Participates in routine chart reviews Attends regularly scheduled in-services and educational conferences. QUALIFICATIONS 1. Must be LVN. 2. Must have a minimum of 1-year Hospice or Home Health Care experience, (ability to use HospiceMD Software) 3. Must possess typing and other clerical skills. 4. Fluent in English/Spanish speaking and writing. JOB TYPE: Full-time Salary: Starting at $60,000 per year PLUS Mileage and Bonuses.
