Home Care Scheduler
JOB SUMMARY:
The Home Care Scheduler is responsible to coordinate and maintain scheduling for The Master’s Touch Home Services, LLC patients and staff. Including the responsibility to ensure that weekly daily documentation is submitted timely and is completed in accordance with the outlined patient care plan. Also ensures timecards are presented to billing timely.
QUALIFICATIONS:
1. H.S. Diploma or HSE; Associates Degree preferred.
2. One (1) year of prior home care scheduling experience preferred.
3. Certified as a Home Health Aide.
4. Computer skills including but not limited to MS Office, MS Excel and Scheduling program.
5. Knowledge of basic medical terminology.
6. Solid interpersonal, organizational and communication skills.
7. Ability to carry out directions, Comprehensive knowledge of the English language.
8. Maturity and ability to deal effectively with the demands of the job.
9. Exhibit outstanding customer service to both patients and employees.
RESPONSIBILITIES:
1. Understands and adheres to the establishment of The Master’s Touch Home Services, LLC policies and procedures.
2. Creates and maintains staff schedules for PA (HHA) and PSA (PCA) employees.
3. Enters scheduling data, creates accurately typed schedules for patients and staff.
4. Ensures that patient schedules are mailed to each patient by the 25th of each month.
5. Schedules staff to cover patient visits according to care plans and staff availability.
6. Responsible to perform a meet and greet for all newly assigned patients.
7. Responsible for following up with patients after they are initially staffed by new caregiver, to inquire about care delivery.
7. Provides staffing for call-ins, PTO and long-term leave.
8. Contacts patients and document as soon as you are aware of staff changes.
9. Responsible to be on call a minimum of fourteen (14) days a month to include after hours and weekends.
10. Enters staff and patient information into database/spreadsheet.
11. Maintains staff and patient database (e.g. patient admission, patient hospital admission/ discharge/address and contact information changes).
12. Maintains staff and patient statistics, and reports, weekly, bi-weekly and monthly.
13. Maintains call -off report log while in the office and on-call.
14. Responsible to be on-call for after hours and weekends; to meet staffing needs.
15. Responsible for completing consumer/patient home visits as needed to become familiar with the patient and their needs. Minimum of six (6) visits monthly.
16. Responsible for performing wellness calls to assigned caseload. A minimum of 25 calls a week should be made.
17. Responsible for personal staff patients as needed when other HHA/PCA staff are unavailable.
18. Collects timesheets weekly for both PA (HHA) and PSA (PCA)
19. Check weekly timesheets/daily documentations against time clock to ensure accuracy.
20. Check weekly timesheets/daily documentations against the outlined client Care Plan to ensure that services are being delivered appropriately; and ensuring compliance with Medicaid and The Indiana State Department of Health.
21. Responsible for ensuring that timecards/daily documentations are checked for completeness and accuracy and ready to be filed away within 72 hours of receipt, if they do not need to be corrected.
22. Responsible for ensuring that completed timecards are delivered to the biller for processing within 48 hours of receipt.
23. Responsible to contact employees within 24 hours to correct daily documentation that has been submitted incorrectly.
24. Responsible to contact patient to verify time worked via phone verification, if daily documentation/timecard is not received timely.
25. Responsible for maintaining a daily contact log of telephone conversation with patients and employees. Documenting call-offs, missed visits, no-call no-shows and tardy visits.
25. Responsible to document and provide the DON a daily record of missed visit.
26. Responsible for informing field staff weekly regarding open and available shifts via email and text messages.
27. Responsible for informing QA when a schedule change requires a change in the care plan.
28. Responsible to actively participate daily in the interdisciplinary staff meetings.
29. Responsible for checking voice mail and email daily and return message within 24 hours.
30. Performs general office duties including but not limited to word processing, photocopies, shreds, filing, sort/distributes mail, provides reception and telephone services, when needed.
WORKING ENVIRONMENT:
Works indoors in The Master’s Touch Home Service, LLC office.
Works in the field - servicing patients in the home as needed and completing required home visits.
