Registered Nurses Needed in Rockford Area

Abcor Home Health is currently looking to hire Registered Nurses to service our homebound patients in the Rockford area in Boone, DeKalb, Ogle and Winnebago counties . This is a per visit position. We can offer you a schedule based upon your availability. This position may require weekends and will include a bonus structure. Ready to work with COVID-19 + patients. Company provides all necessary PPE. Position Summary: To provide nursing care in the patients’ home on an as needed basis in accordance with the patient's plan of care. This includes comprehensive health and psychosocial evaluation, monitoring of the patient’s condition, health promotion and prevention coordination of services, teaching and training activities and direct nursing care. Responsibilities: -Perform admission, transfer, re-certification, resumption of care and discharge OASIS for the home care patient -Consult with the attending physician concerning alterations of Patient Care Plans, checks with the appropriate supervisor and makes changes, as appropriate -Submit clinical documentation as directed by Director of Nursing in a timely manner -Discuss with the appropriate supervisor the need for the involvement of other members of the health team -Work closely with admission clerk to coordinate patient admission -Participate in the patient’s discharge planning process -Establish the patient's individualized treatment and medical record baseline -Collaborate with physicians, other agency staff and contract services to coordinate and implement the plan of treatment. Re-evaluate the plan on an ongoing basis and report condition changes Qualifications: -Must be a registered nurse with the Illinois Department of Professional Regulation -Must have a minimum of one year hands-on clinical experience -Prior Home Health experience is preferred -Preferred knowledge of Kinnser/Wellsky Software -Preferred wound care certification -Must have a driver’s license and be willing and able to drive to patient’s residences -Bi-Lingual Spanish is preferred This position operates out of our Rockford office: 127 N. Alpine Road, Rockford, IL 61107. Salary: Pay: $40.00 - $60.00 per visit
