Luthas Health Care is seeking skilled and compassionate Registered Nurses (RNs) to join our home health care team, serving clients across Barnstable County. If you’re looking to make a meaningful impact while enjoying a flexible schedule, we’d love to hear from you.
Key Responsibilities:
-Perform in-home clinical assessments and skilled nursing visits
-Develop and update individualized care plans
-Administer medications, wound care, injections, and manage treatments
-Monitor patient conditions and maintain accurate EMR documentation
-Educate clients and families on chronic conditions and safety practices
-Coordinate care with physicians, therapists, and home health aides
-Support clients with memory care, developmental disabilities, or DME needs
Requirements
-Active Massachusetts RN license
-At least 1 year of nursing experience (home health preferred)
-Ability to work independently and travel within Plymouth & Barnstable Counties
-Proficient in EMR documentation and mobile charting
-Reliable transportation and valid driver’s license
