Job Responsibilities
Perform Start of Care (SOC), Resumption of Care (ROC), Recertification, Follow-up, and Discharge visits.
Conduct comprehensive nursing assessments and develop individualized care plans.
Provide skilled nursing services, including wound care, medication management, disease management, catheter care, injections, and patient education.
Monitor patient progress and promptly report significant changes in condition to the physician and clinical team.
Complete OASIS assessments and documentation accurately and in a timely manner.
Coordinate care with physicians, therapists, caregivers, and other healthcare professionals.
Educate patients and families on disease management, medications, safety, and preventive care.
