Job Description: Responsibilities: • Conduct comprehensive client assessments to determine individual care needs. • Develop and implement personalized care plans based on assessment findings. • Perform regular follow-up assessments to monitor client progress and adjust care plans as needed. • Create and maintain caregiver competency checklists to ensure caregivers are meeting required standards. • Conduct competency verifications for caregivers, including observations and assessments. • Provide training and support to caregivers to enhance their skills and knowledge. • Ensure compliance with all regulatory requirements and company policies. • Communicate effectively with clients, families, and other healthcare professionals to coordinate care. Qualifications: • Current and valid Registered Nurse (RN) license in the state of North Carolina. • Minimum of 1 year of experience in home care, healthcare, or a related field. • Strong clinical assessment and critical thinking skills. • Excellent communication and interpersonal skills. • Ability to work independently and as part of a team. • Proficiency in using electronic health records (EHR) systems. • CPR certification is required.
