**Job Description:**
We are seeking a compassionate and dedicated Hospice Registered Nurse (RN) to join our team. In this role, you will provide high-quality care and emotional support to patients and their families, ensuring comfort, dignity, and respect for each patient. As part of our hospice team, you will conduct assessments, develop care plans, manage pain and symptoms, and collaborate with other healthcare professionals to ensure the best possible care in patients' homes, hospice facilities, or long-term care settings.
**Responsibilities:**
- Conduct comprehensive patient assessments and develop individualized care plans
- Administer medications, therapies, and treatments in line with the care plan
- Provide education and emotional support to patients and their families
- Collaborate with a multidisciplinary team to ensure coordinated care
- Manage pain and symptom control while maintaining patient dignity
- Monitor patient condition, progress, and adjust care plans as needed
- Maintain accurate and up-to-date documentation in compliance with hospice standards
- Participate in regular team meetings and provide insights for continuous improvement
**Qualifications:**
- Current Registered Nurse (RN) license in [Specify State]
- Minimum of 1 year of experience in hospice, palliative care, or related field preferred
- Compassionate and empathetic, with strong communication skills
- Ability to work independently and as part of a team
- Valid driver’s license and reliable transportation
- Basic Life Support (BLS) certification required; Advanced Cardiovascular Life Support (ACLS) preferred
