Summary Guaranteed is seeking to add passionate and empathic Hospice Nurses to our growing Hospice Care Teams across the Los Angeles region. At Guaranteed, our Registered Nurse sees the value of hospice care in alleviating pain and suffering; and, more so, to support our patients in having a dignified death. Our Nurses aim to provide excellence in care and support for end-of-life patients and their families. As a Hospice Nurses, you will plan, organize and direct hospice care with emphasis on communal health education. You will build upon organizational resources to plan and direct services to meet the needs of individual and families within their homes and communities. Job Responsibilities Complete an initial, comprehensive and ongoing comprehensive assessment of patient and family to determine hospice needs Develop a care plan that establishes goals, based on nursing diagnosis and incorporates palliative nursing actions. Includes the patient and the family in the planning process Initiate appropriate preventive and rehabilitative nursing procedures. Administers medications and treatments as prescribed by the physician in the physician’s plan of care Assist the patient and their families with health care instructions and the activities of daily living to facilitates the patient’s efforts toward self-sufficiency and optional comfort care Complete, maintain and submit accurate and relevant clinical notes regarding patient’s condition and care given; and communicates this information with community health related persons to coordinate the care plans Additional Duties Attend the Interdisciplinary Group Meeting bi-weekly Participate in on-call duties as defined by the on-call policy Ensure arrangements for equipment and other necessary items and services Supervise ancillary personnel and delegates responsibilities when required Actively participates in quality assessment performance improvement teams and activities Complete virtual visits, as needed Skills and Toolset You’ll Need to Bring Knowledgeable in caring for the terminally ill and performing tasks such as administering medication and providing emotional support A compassionate nature and excellent interpersonal skills Self-directed and able to work with minimal supervision Experience teaching patients and caregivers self-care techniques, medication administration, and dietary instructions in concert with the interdisciplinary group of care providers Background in providing continuity of care in cooperation with hospice interdisciplinary group to meet the physical, spiritual, and emotional needs of the patient and caregivers Excellent observation, verbal and written communication skills, problem solving skills, basic math skills; nursing skills per competency checklist Desire to assumes responsibility for personal, professional, and practical skill growth and development through attendance and participation in continuing education and in-service classes Relevant Experience Bachelor of Science in Nursing (BSN) and current RN license and CPR certification Minimum of two years’ experience as a professional nurse, at least one of which is in the area of home care or hospice is required Must be a licensed driver with an automobile that is insured in accordance with state and/or organization requirements and is in good working order Physical conditions: Ability to handle prolonged standing or walking Ability to lift, position and/or transfer patients Ability to lift supplies and equipment Ability to complete considerable reaching, stooping bending, kneeling and/or crouching Visual acuity and hearing to perform required nursing skills 3 years experience as a lead hospice RN Acts as Case Manager when assigned by Clinical Supervisor and assumes responsibility to coordinate patient care for assigned caseload Management experience is preferred; responsible for supervising hospice aides and the LVNs Experience conducting virtual visits is a plus
