Licensed Vocational Nurse (LVN) Job Description
Summary Guaranteed is seeking to add passionate and empathic Hospice Licensed Vocational Nurses (LVN) to our growing Hospice Care Teams across the Los Angeles Region. Our Hospice LVNs will provide exceptional care and comfort to those in their end-of-life journey under the supervision of a registered nurse. The Hospice LVN is responsible for developing plans for individual patients to ensure they receive the care they need and communicates with family members and/or caregivers regularly. Job Responsibilities Implement current nursing practice following the individualized plan of care during each visit whether in person or virtual Provide accurate and timely documentation of patient services to reflect the plan of care Assess and provide patient and family/caregiver education and information pertinent to diagnosis and self plan of care Participate in coordination of hospice services, appropriately reporting the identified needs to the interdisciplinary group (i.e., hospice aide, OT, PT, SW, ST, Dietician or Clinical Supervisor) Provide appropriate pain/symptom management. Evaluates patient’s response to treatments/medications. Participate in Guaranteed’s quality assessment performance improvement teams and activities Skills and Toolset You’ll Need to Bring Deep understanding of the philosophy of hospice Empathic of the needs of the terminally ill, family, and caregivers Experience with use and preparation of equipment and supplies effectively, adhering to aseptic technique Demonstrates good verbal and written communication, and organization skills Drivers license and access to transportation in good working condition with current insurance Possess and maintain current CPR certification Currently licensed as an LPN/LVN in respective state of application Desire to assumes responsibility for personal, professional, and practical skill growth and development through attendance and participation in continuing education and in-service classes Nice to Have 3 years of Hospice experience (LVN, CNA, etc.) Experience conducting virtual visits Relevant Experience Graduate of an accredited practical nurse or vocational nursing program Minimum 3 years nursing experience Community health/hospice or medical/surgical experience is required
