Caring supportive management
Registered Nurse Position:
Coordinates and provides rehabilitation health services for post acute and post rehab patients in community setting
Works autonomously in flexible day shift schedule assessing physical, functional, and psychosocial needs of patients
Identifies plan of care, providing, and coordinating medical and rehabilitation services for patients
Collaborates with multi-disciplines including Medical/Psychiatric Practitioners, Physical Therapists, and Dietician
