Assess, generate, and evaluate patient care plans and patient progress. Liaise with other members of each patient's multi-disciplinary care team (e.g. Physical therapists, social workers, etc.) Record patient progress in HCHB via tablet. Attend regular staff meetings. Job Types: Full time and Per Diem ********************************* Requirements: Current NJ RN License Reliable transportation, Driver’s License, and Insurance. Strong interpersonal and customer service skills. Ability to demonstrate knowledge of electronic documentation (HCHB or others) Bilingual a plus Additional training certificates a plus.
