Responsibilities: Providing limited term skilled nursing services to individuals returning home from higher level care facilities Assessing, generating, and evaluating patient care plans and patient progress Liaising with other members of each patients multi-disciplinary care team Recording patient progress in HCHB via tablet Attending regular staff meetings Requirements: Current NJ RN license Reliable transportation, drivers license, and insurance Strong interpersonal and customer service skills Ability to demonstrate knowledge of electronic documentation Additional training certificates a plus Preference for nurses with prior Home Health experience including: Psych, Vent, Peds, Admissions Job Types: Full-time, Part-time, Travel nursing Standard shift: Day shift Weekly schedule: Monday to Friday Rotating weekends Experience: Nursing: 1 year (Preferred) License/Certification: RN License (Preferred) Work Location: On the road
