Registered Nurse (RN) Job Description
Another Day at Life Home Health Care is seeking an experienced Registered Nurse (RN) with case management skills to serve clients in the Kansas City, Missouri area.
We are looking for an RN who understands the coordination of care and is comfortable working with clients receiving services through Missouri Medicaid/MO HealthNet and Home and Community-Based Services (HCBS). The ideal candidate will have experience with client assessments, care planning, documentation, and ongoing monitoring of client needs.
Responsibilities
Provide nursing oversight and case management for assigned clients
Complete client assessments and ongoing evaluations as required
Review and assist with implementation of client care plans
Monitor changes in client condition and service needs
Coordinate care and communicate with clients, families, caregivers, providers, and other members of the care team
Maintain accurate and timely documentation
Provide oversight and support to caregivers/personal care staff as appropriate
Assist with ensuring services are provided according to authorized plans of care and applicable requirements
Communicate changes or concerns that may require updates to client services or care plans
Qualifications
Current Registered Nurse (RN) license in Missouri in good standing
Case management skills required
Experience with Missouri Medicaid/MO HealthNet, HCBS, in-home care, or community-based services strongly preferred
Experience with client assessment, care plans, service coordination, and clinical documentation
Strong organizational and communication skills
Ability to work independently and coordinate effectively with clients, families, caregivers, and other professionals
Experience working with elderly individuals, individuals with disabilities, and/or individuals with medically complex conditions preferred
Location: Kansas City, Missouri area
