Part time Registered Nurse and administrator

• Assist in creation of plan of care for all new individuals served. • Conduct assessment and preadmission of new clients. • Serve as liaison between clients and company. • Conduct supervisory visits and document results. • Complete and update health, risk and safety assessments using the Health Risk Screening Tool (HRST) • Review of data tracking and documentation • Conduct proxy care training. • Provide staff training and oversight. • Supervision of CNA, PCT, Caregivers, and LPN Staffs • Coordinate medical visits for clients. • Fill in on medication passes as necessary. • Other duties as assigned. • Report directly to the Directory of Nursing
